The form below is used to submit the Volunteer of the Month and Annual Awards (Friend of NAPO-NE and Service to NAPO-NE) information to the Web Chair for posting. If a Volunteer of the Month is not selected for any month, submit a notification form and select the “No Volunteer of the Month” this month option.
Please note the following:
- The Volunteer of the Month will be posted to the web site by the end of the week it was submitted. Please check and notify email@example.com if your submission is missing.
- Photo: The submitted photo needs to be of the Volunteer of the Month recipient ONLY. If the member is not at the meeting when the award was announced, you need to obtain the photo. If you need to obtain a photo, either contact the recipient and have them email you a photo so you can submit it on the form or download their Chapter website profile photo and upload it to the form. Note: The photo is required in order to submit the form.
- Statement of why the individual received the award: This can be very brief but gives enough information for the reader to know why the individual was singled out of all our volunteers.
Here’s an example from January 2017:
“Thank you, Heidi Solomon, for thinking of and volunteering for a “mini project” of helping members to update their NAPO-New England website profiles — a very important project!”
